Ideally COGS recipients will complete accountability online.
If you require a paper copy of the accountability report:
• please do not use the accountability report on your client home page as it is out of date
• please download the new accountability report from the Available Grants section accessed from the Grants Online home page. Complete the report, sign the declaration, and mail to this office.
